Recertification

February 22, 2008

Dear Rehabilitation Doctor:

Once again it is time to recertify with the American Chiropractic Rehabilitation Board.  In order for the board to keep a doctor active on the registry, a yearly recertification process needs to be completed.  This process includes submitting answers to questions, which are related to rehabilitation articles posted on our website.

Enclosed you will find the recertification renewal form.  The form and recertification fees must be received in our office by October 1, 2008.  Any forms received after this date may be subject to a late fee.  Once the form and fees are received, you will be sent an instructional e-mail detailing the process to complete the questions.  When they are successfully completed (75% or better), a registry certificate will be sent to you.  If you do not receive at least 75%, you must retake the questions and a $50.00 fee will apply.  If you have not recertified in the past or have not recertified for two consecutive years, please contact the office to determine the steps that need to be taken in order for you to become active again.

If you successfully complete any written or oral exam within a given year, you are not required to pay the recertification fees, nor are you required to complete the questions.  If you are planning to take an exam this year, you may delay sending in the form and fees.  If you do not pass the exam, you will need to send in the recertification fees and complete the questions.

We will continue to offer four continuing education credits for completion of the recertification process.  Please be sure that your state or province accepts online continuing education credits prior to requesting this on the form.  Please go to www.chirocredit.com, under state approvals, to see which states allow distance learning credits.  We will apply to those states for the credits at no additional cost to you.

Please contact the office if you should have any questions once this letter is received.  Thank you for your time, consideration, and support.

Sincerely,

Thomas Fowler, DC, DACRB

President, American Chiropractic Rehabilitation Board
